A simple nod of the head can mean several things. It can mean tiredness, so much so one can not hold one's head upright. It can mean sleepiness. It occurs when one is about to drop off to sleep or unconsciousness. Or it can mean a subtle acknowledgement or agreement. So there is no one cardinal rule for the act of nodding one's head.
